What is a wisdom tooth extraction (or removal)?

Wisdom tooth removal is a surgical procedure to extract one or more of the four permanent adult teeth at the back corners of your mouth, known as third molars. These teeth often lack sufficient space to grow properly, leading to impaction and potential dental issues such as pain, infection, overcrowding or damage to nearby teeth. Even if they aren’t causing immediate problems, dentists may recommend removing wisdom teeth to prevent future complications.

What is a tooth extraction?

Tooth extraction is a dental procedure that removes a tooth from its socket in the jawbone. Often called “pulling” a tooth, this process is necessary when a tooth is damaged beyond repair, poses a risk to other teeth, or contributes to oral health issues. Whether due to decay, trauma, or overcrowding, tooth extraction ensures the health and alignment of your remaining teeth.  

When do you need a wisdom tooth extraction?

Wisdom teeth are the last set of permanent teeth to emerge, typically between the ages of 17 and 25. While some people experience no issues as their wisdom teeth come through, many others face complications due to insufficient space in the mouth, leading to impacted wisdom teeth.

Impacted wisdom teeth fail to erupt properly due to lack of room. They can grow at various angles: towards the next tooth (the second molar), towards the back of the mouth, at a right angle to other teeth (lying down within the jawbone), or they may grow straight but remain trapped within the jawbone. These irregular growth patterns can cause pain, infection, and damage to adjacent teeth, necessitating removal.

While healthcare providers aim to preserve natural teeth whenever possible, there are situations where extraction becomes necessary. Removal may be the best option if a tooth is too damaged for restorative treatments like dental inlays or onlays and crowns. Tooth extraction might be recommended if you have:

  • Severe tooth decay (cavities).
  • A fractured tooth.
  • An impacted tooth.
  • Crowded teeth.
  • Severe gum disease.
  • Tooth luxation or other dental injuries.

The Tooth Extraction Process

Before the Procedure

Before a wisdom tooth or standard tooth extraction, you’ll meet with your Cape Town dentist to discuss the procedure. During this consultation, you should:

  • Discuss the type of anaesthesia to be used: local, IV sedation, or general anaesthesia.

  • The surgery typically takes 45 minutes or less, depending on how many wisdom teeth require extraction.
  • Anesthesia options include:
    • Local anaesthesia: Numbs your mouth, and you may also receive nitrous oxide (laughing gas) to help you relax.
    • IV sedation: Provides drugs through a vein to make you drowsy, possibly causing you to sleep through the procedure.
    • General anaesthesia: Ensures you are asleep and unaware during the surgery.
  • If necessary, the surgeon will make incisions in your gums or bone to access and remove the teeth, followed by stitches to promote healing.
Standard Tooth Extraction:
  • Local anaesthesia is administered to numb the tooth and surrounding gum tissue.
  • The dentist uses specialised instruments to loosen and lift the tooth from its socket gently.
  • In some cases, incisions in the gums are needed to access the tooth, significantly if it is decayed or broken off at the gum line.
  • After removal, the socket is cleaned and disinfected. A dental bone graft may be placed to prevent bone loss.
  • Stitches may be applied to aid in healing.

Recovery from the wisdom tooth and standard tooth extractions varies based on the type of anaesthesia and individual response.

Immediate Aftercare:
  • If you had local anaesthesia and felt alert, you could drive home. Otherwise, you’ll need someone to drive you.
  • Post-surgery discomfort is typically minimal, with swelling and mild discomfort for a few days.
  • Follow your doctor’s instructions for a smooth recovery.

For the first three days after surgery:
  • Do:
    • Use an ice pack on your face to reduce swelling.
    • Apply moist heat for a sore jaw.
    • Gently open and close your mouth to exercise your jaw.
    • Eat soft foods like pasta, rice, or soup.
    • Drink plenty of fluids.
    • Brush your teeth from the second day, avoiding any blood clots.
    • Take prescribed medications to ease pain or swelling.
    • Contact your doctor if you have a fever or pain and swelling persists.
  • Don’t:
    • Avoid drinking through a straw to prevent dislodging blood clots.
    • Don’t rinse your mouth harshly; use gentle saltwater rinses if advised.
    • Refrain from eating hard, crunchy, or sticky foods that could irritate the extraction site.
    • Do not smoke, as it can slow the healing process.
