So Dr Yudelman, what are realistic cosmetic goals and when are perfect Hollywood teeth not worth the cost?

That’s a great question to start off with. So we did a podcast, I think it was number 47, Hollywood versus real life smiles. I always advise setting your goals around a natural, healthy and confident smile instead of chasing those perfect veneers that you see in magazines.

I’ve seen many patients regret very aggressive treatments, motivated by trends or social media. The truth is big changes are permanent and introduce more risk, like sensitivity or bite issues, and they’re costly not only in what you pay but also in replacement cost and long-term biological cost. If they have to file away some or all of your tooth to make your teeth look better, it’s just not worth it long term. If they’re done really well and minimally invasive, veneers can last 10 to 20 years, but if you go off to Turkey to get ‘Turkey teeth’ or you go to a local dentist offering cheap cosmetic work, it’s not worth it because they might only last two or three years and then what’s left of your tooth is compromised. Most smiles just need subtle whitening or a little bonding and it can be truly life-changing. Less is more — for your looks and your bank balance. It’s your own face, your personality, your lifestyle, and that’s what genuine cosmetic success looks like. Unless you’re on stage and people need to see your teeth from a kilometre away, most people are right up next to you. Just be natural.

Okay, and how do you compare at-home whitening kits to professional treatments for cost and results?

We did several podcasts about whitening, including number 32 about take-home kits. At-home kits are great for removing stains, budget-friendly, convenient, and they can take a while — you can whiten overnight or for several months. At OptiSmile, we don’t use overnight gels. We 3D print trays and use seven and a half or nine and a half percent hydrogen peroxide instead of carbamide peroxide. You use it for about half an hour a day, and within a week you’ll see dramatic results.

You can do in-chair whitening as a boost, but the result is the same in the end. Once your teeth are white, they’re white. In-chair saves time but costs more, and you always need a professional cleaning and full check-up first. People who go to non-dental whitening places often don’t get proper exams, and they may have fillings or veneers that won’t match. Take-home trays are also great because you can top up every few months or before big events.

Let’s talk aligners like Invisalign. Are all aligners really the same quality or should you be cautious with lower-priced brands?

At the upper end, international brands like Invisalign have huge quality control and training. Becoming a provider involves extensive certification, ongoing courses and digital planning. The training is often more important than the brand. We stick with Invisalign, though we’re starting to 3D print our own aligners too. Invisalign has years of research, oversight, and user-friendly software. We now get treatment plans back in minutes, which is incredible. We’ve seen cases where cheaper aligners caused problems and we had to correct them. Always choose a provider with expertise and transparent planning. It’s your smile — it’s worth it.

I just want to quickly get back to whitening. Is in-chair whitening ever too much of a good thing?

Your teeth can only go as white as they naturally can. That blinding white look usually comes from thick veneers or crowns. In podcast 34 we spoke about moderation — using gels properly. If the dentist or hygienist is trained in the system, in-chair whitening will follow strict protocols. At OptiSmile, we don’t use a full four-treatment kit; we split it, which lowers cost and sensitivity. Some heavily advertised systems can cause painful ‘zingers’ — shooting pains in the lower front teeth for days. Patients never forget it. My brother used to install electric fences in South Africa before moving to Australia. I asked if they kill you. He said no, but you’ll never forget the shock. It’s the same with bad whitening experiences. One of those brands sounds like you’re going fast — maybe too fast!

When it first came out, I felt confident it would last, and now six years later it’s proven itself. We even put a five-year warranty on it. If someone chips it, we can repair it easily. Always ask what’s included in the price, what happens if things go wrong, and whether digital planning is included. We now use Smile Cloud with AI so patients can see their smile design before any work starts. Surprises are for parties, not dental care! Make sure you’re on that journey with the dentist — not watching them on theirs.

Yes, absolutely. In our podcast on bonding, we discussed how small changes — like closing a gap or gentle recontouring — can make a massive difference in one painless visit. No drilling, no major cost. Less is more. I had a patient last week who had Invisalign and whitening but still wasn’t happy. She had long, thin teeth. I explained cosmetic contouring, where we gently polish the edges of the enamel if there’s enough structure. Even half a millimetre makes a big visual change. I marked her teeth with a pencil to show her what I’d do. We shortened the two front teeth slightly, polished, rounded the corners, and she was thrilled. It took minutes and we didn’t even charge her. The transformation was incredible.

What’s the truth about online DIY orthodontic and veneer kits?

That’s a big trend overseas, but most of those mail-order aligner companies have failed. There are many news stories about people being left mid-treatment with ruined teeth. Even though aligners seem simple, they need professional supervision. There are also ‘veneer technicians’ doing dentistry illegally. Make sure your provider is registered. You only get two sets of teeth — baby and adult. There are no bargains in dentistry or brain surgery.

Great question. At OptiSmile, we follow the three S’s: situation, shade, and shape. First, align the teeth with Invisalign or clear aligners (situation). Next, whiten to improve the shade. Finally, refine the shape or size with bonding or contouring. We now do digital 3D scans and injection moulded composite bonding using custom moulds and AI planning. The results are natural and predictable. Veneers can look artificial unless done perfectly, but injection-moulded composites often look more like real teeth. We’ll soon do a podcast about 3D printed fillings and veneers — that’s the future.
